"Just Checking improved the quality of assessment and outcomes for the service users", concluded Leeds at the end of a 1 year pilot in its mental health intermediate care service.
Meeting Objective 9 of the National Dementia Strategy - Improving Intermediate Care - the staff at Leeds report that Just Checking:
- provided more opportunities to support people at home
- helped to delay the need for residential care
- prompted earlier hospital discharge
- improved capacity in the intermediate care team
